Hi, I've been taking the maximum dose of Mirtzazepine for over a year and before that, 30mg for 6/7 years. I've just seen a psychiatrist for the first time who suggested coming off it due to the nightmares I was having. I asked her if it would be tapered which she confirmed, but I've just found out my dose is going to be reduced to 15mg for two weeks and then nothing. I've very worried that 2 weeks to taper off may not be safe. I have a benign brain tumor that causes tonic clonic generalised seizures that have been witnessed to as seriel seizures twice, and I'm worried that such a fast withdrawal may put me in danger of having a seizure. I live alone. Do you have any advice for me?

hi Conan I noticed that your institution sends urinary strep pneumoniae and legionella antigens as well. I personally feel that these tests are rather useless, especially strep pneumoniae. most of my patients are already empirically started on augmentin on admission so the result of the test won't actually change management. legionella rarely comes back positive especially if it's a community acquired pneumonia. so I might do a legionella if my patient isn't improving despite a few days of antibiotics or is immunocompromised. what do you think?

One app that actually worked for me is called AppBlock. I set social media limit to 1 hour a day. I give my wife the password so only she knows how to bypass it. Has helped cut back my scrolling a ton :)

As far as the eating healthy part goes, I changed my diet a while back and I could not believe the effect it had on how I feel. I threw out all the cereal, sauces, process food and soda. It was pretty hard core to throw things out because it's against my frugal nature, but I think it helped. I then started buying only: steak, eggs, chicken, bacon, broccoli, plain yogurt, nuts, brown rice, potatoes, sweet potatoes, peppers, onions, and a few bananas, apples and oranges. I might buy something else occasionally, but if it isn't natural, I don't buy it. I also only drink coffee, water, and milk. I usually only eat the carbs (rice, potatoes, bananas, etc) before I do some sort of physical activity. The result has been I feel better all around and have lost body fat around the middle. I also have less inflammation and mucous generation in my body. Now if I go out with friends to eat and cheat by eating processed food or have soda, my body tells me. Last week I ate some things at a fast food joint on a trip and that night my body made tons of flem, I got a headache, and my back was tight. I also have had good blood test numbers recently, so I'm not worried about cholesterol.

Yeah. The other thing of it is that in my experience the split is often predicated on their expectations of care. Expectations of care are typically unreasonably high and as soon as you can't meet them (or refuse to due to unreasonableness) they often will split the other way in my experience. Not meeting their expectation for them triggers up anxiety around abandonment which causes them to lash out. For me that's always the hardest part of taking care of BPD patients, the knowing that as soon as I don't give them exactly what they want they're gonna be mean. The only thing you can do to sort of counteract this is set really, really strict boundaries around their expectations care from the get-go, it doesn't work great even then. As you might guess I have a couple in my clinic right now I'm getting a little transference from :D

Experts i.e. people who actually pay attention to things like the economy will also point out, economic successes or failures of each administration is an end result of the cumulative decisions made by the previous administration. People need to realize the tools at each president's disposal are limited when it comes to the economy & are geared to being reactive v. proactive i.e. correcting mistakes instead avoiding mistakes all together. That's NOT to say a very poor President (or poor businessman) can't tank the economy while still in office but that any changes they made won't fully mature until they are no longer in office. Thus a President frivolously taking credit for an uptick in the overall economy is as foolish as a President taking credit for a sunny day... it's more correlation than causation. President's make decisions that'll impact tomorrow not today.
